Community Wellness and Safety of Arizona

Area Served: Locations in Gilbert and Phoenix
Eligibility: Open to the general public age 15 and older. Thimerosal-free vaccines are available for pregnant women, nursing mothers, and anyone with allergies to Thimerosal. Pregnant women must have a written consent from their doctor to recieve a flu shot.
Availability: Through June 2013

Fees: $28

No insurance plans accepted, but receipts are available for patients to submit to their insurance providers for possible reimbursement.

On-site flu vaccine clinics are available for groups of 10 or more. Call 480-325-9459 for information.

CVS

Area Served: Statewide
Eligibility: Open to the general public age 18 and older
Availability: Check with your local CVS

Fry's

Area Served: Statewide
Eligibility: Call your local Fry's pharmacy for information
Availability: While supplies last

Healthwaves

Area Served: Statewide

Eligibility: Open to the general public age 4 and older

Availability: August-January 2013

Mollen Immunization Clinics

Area Served: Multiple locations statewide

Eligibility: Open to the general public age 4 and older

Availability: Clinics staffed by professional nurses and open August through December 2012

Most insurance plans accepted. No co-pay, no out-of-pocket expense for Medicare Part B. Co-pays vary for other insurance plans with most requiring no co-pay. Check here for details.

Walgreens

Area Served: Statewide
Eligibility: Open to the general public age 4 and older
Availability: Check with your local Walgreens or call 877-331-9842 to check availability or schedule an immunization appointment.

Insurance Plans Accepted

  • May be covered for eligible Medicare Part B patients who are not members of an HMO. Walgreens will submit a claim to Medicare on your behalf. A Medicare card must be shown when services are provided.

If your health insurance plan covers the cost of your seasonal flu shots, you must present your insurance card at the time of service. If you have an issue relating to your medical insurance or a health-related issue with a flu shot clinic, please contact your insurance provider and/or the clinic for assistance.
