Eligibility: Open to Coconino County residents age 6 months and over.
Cost: $25 for adults, free for uninsured children age 18 and under and for children whose insuarnce does not cover immunizations
No copay with the following insurance:
 AHCCCS
BlueCross BlueShield of Arizona (excluding Medicare and out-of-state BlueCross BlueShield plan members)
Health Choice Arizona
Medicare
Phoenix Health Plans
